What Are Dermal Fillers?

Faces lose volume as they age. It happens quietly, over years, and it is often why people look tired or drawn without being able to say exactly what has changed. Fat pads in the cheeks slip downwards. Lips thin. The jawline softens. The folds either side of the nose and mouth deepen because there is less support underneath them.

Dermal fillers replace that lost support. Most are made from hyaluronic acid, a substance your skin already produces, which binds water and gives tissue its cushion and bounce. Injected in the right plane, in the right amount, it lifts and hydrates where volume has gone.

The word “filler” is slightly misleading. The best results rarely come from filling a line directly. More often we treat the area that has lost support, and the line softens as a consequence. A deep smile line, for example, is usually better treated by restoring the cheek above it than by injecting the fold itself.

Areas We Treat

Lips

 Added volume, better definition, or simply correcting an imbalance between the top and bottom lip. Most of our lip work is conservative, and we would rather build gradually over two appointments than overfill in one. Read more about lip fillers →

Cheeks

Restoring the volume that sits high on the cheekbone. This is often the most effective single treatment for someone who looks tired, because lifting the midface improves the area below it at the same time.

chin

A small, recessed or slightly off-centre chin affects the balance of the whole lower face, and in profile it is often what makes a nose appear larger than it is. Chin filler can bring the profile into proportion.

Jawline

Definition along the jaw and a cleaner line between the face and the neck. Suits people whose jawline has softened with age, and those who have always wanted more structure there.

Nasolabial Folds

The lines running from the nose to the corners of the mouth. These soften considerably with treatment, though as above, we often treat the cheek to achieve it.

Tear Troughs and Temples

Hollowing under the eyes and at the temples. These are more advanced areas and suitability is assessed carefully — not everyone with under-eye shadowing is a candidate, and we will tell you honestly if you are not.

What Happens at Your Appointment

Your consultation comes first and it is free. We look at your face as a whole rather than at the one feature you came in about, because treating a single area in isolation is how results start to look obvious.

You will be asked what you want to change and, just as importantly, what you want to keep. We take a full medical history, discuss your previous treatments if you have had any, and explain exactly which product we would use, where, and how much.

Most filler products contain a local anaesthetic, and we usually apply numbing cream beforehand as well. Treatment takes between fifteen and forty-five minutes depending on the areas involved. You will see a change immediately, though this is not the final result — some initial swelling is normal and settles over the following days.

Lip treatments in particular look fuller on the day than they will at two weeks. We tell every client this before they look in the mirror.

Results and Aftercare

How Long It Lasts

Between six and eighteen months, depending on the area and the product. As a rough guide:

  • Lips: six to nine months
  • Cheeks: twelve to eighteen months
  • Jawline and chin: twelve to eighteen months
  • Nasolabial folds: nine to twelve months

Areas that move a lot, like the lips, break the product down faster. Your metabolism and how active you are make a difference too.

Honest expectations

fillers add volume. They do not tighten loose skin, and they do not replace a facelift. If significant skin laxity is the real issue, filler alone will not fix it, and adding more product makes faces look heavy rather than lifted. We would rather tell you that at consultation than take your money.

Aftercare

For the first 24 to 48 hours:

Avoid strenuous exercise, saunas and sunbeds

Skip alcohol for 24 hours, as it worsens bruising

Stay out of extreme heat and cold

Do not massage the area unless we have specifically told you to

Avoid dental work for two weeks where possible

Swelling, tenderness and some bruising are normal, particularly with lips. Swelling usually peaks around 24 to 48 hours and settles within a week. Arnica and a cold compress help.

Contact us immediately if you develop severe or increasing pain, if the skin turns pale, white or mottled, or if your vision changes. These are rare but they need urgent attention, and we would always rather hear from you unnecessarily.

Answers to the questions clients ask most often before treatment.

Does it hurt?

Less than most people expect. We use numbing cream, and the products themselves contain local anaesthetic. Lips are the most sensitive area, but it is manageable and over quickly.

Done well, people notice that you look well rather than that you have had something done. That depends entirely on the amount used and the practitioner’s judgement, which is why we treat conservatively and build gradually.

You can go back to work the same day. If you have an event coming up, book at least two weeks beforehand so any swelling or bruising has fully settled.

Hyaluronic acid fillers can be dissolved with an enzyme injection if necessary. It is not something we want to use, but it means the treatment is reversible, which is one of the reasons we work with these products.

Yes, and many clients do. They do different jobs — anti-wrinkle injections relax muscle movement, fillers restore volume. We will explain which one actually addresses your concern, and sometimes the answer is only one of them.

We treat adults aged 18 and over only. We cannot treat you during pregnancy or breastfeeding, if you have an active skin infection in the area, or where certain medical conditions or medications make it unsuitable. This is assessed at consultation.

Often, yes. Bring details of what was used if you have them. We will give you an honest assessment, which sometimes means dissolving the existing product and starting again rather than adding more.

We cannot answer that before seeing you, and anyone who quotes you a number over the phone is guessing. It depends on the area, the degree of volume loss, and the result you want.
